Not sure how old this bug is, but I recall back in NC 1 it would actually show peoples names, if its no longer possible, just label them 'dogtag'

i don't think this is a bug but rather an optimization? database queries are expensive.

Happens usually when the char was deleted.

Definitely not deleted chars, as these were fresh kills.
